Young Adults Service

Action for Children and Derbyshire Federation for Mental Health are working in partnership to develop and deliver transition services for young people aged between 18 and 25 with a mental health issue. The service is currently aimed at young people who are being supported by the Children and Adolescent Mental Health Service (CAMHS) and are due to be discharged, but who have continuing support needs.

The service is being piloted in Derbyshire (Chesterfield will be the first locality). Rollout will be undertaken in 3 phases:

Phase 1 – CAMHS Direct Referrals (a step down from CAMHS)
Phase 2 – Social Care, Leaving Care, Direct Referrals
Phase 3 – Self Referrals and referrals from other organisations such as GPs and Universities etc
