Support Questionnaire - for 3 to 7 year old children

The support questionnaire needs to be completed for all children registered with Bucks Activity Project. The questionnaire determines the level of offer available to each individual child. You only need to complete one questionnaire, which depends on your child's age. If your child is under 3, you do not need to complete a questionnaire just yet, just send us your completed registration form. If your child turns 8 in the next 3 months, please complete the 8 - 19 year old questionnaire.
